Job Description
A medical based company is looking to fill a 6-month contract to hire Community Health Worker position in Irvine, CA
Pay Rate : $30/hr (Paid weekly) Mileage is reimbursed as well. Radius about a 25-30 miles
Work Schedule : Monday - Friday, 8am - 5pm
Allmed Benefits : Dental Insurance, Health Insurance, Vision Insurance and 401(k)
Contract to Hire:
The Field-Based Community Health Worker (CHW) supports members' health and independence by conducting home visits, assisting with care transitions, and addressing social determinants of health (SDOH).
Primary Responsibilities:
Health Education and System Navigation
Educate individuals and families on health conditions, preventive care, and healthy lifestyle choices
Help clients understand medical information and navigate the healthcare system
Client Advocacy
Advocate for clients by connecting them with essential resources such as food, housing, and medical care
Promote culturally responsive services and work to reduce barriers to care
Community Outreach and Data Gathering
Conduct outreach activities, including home visits, to identify health needs and connect individuals to services
Collect and report community health data to inform health officials and service providers
Emotional Support and Informal Counseling
Provide empathetic, non-judgmental support and build trusting relationships
Offer informal counseling and social support to empower individuals in managing their health
Serve as a liaison between community members and health/social service providers
Coordinate services and improve communication to ensure comprehensive client care
