Job Title: Home Health Case Manager • Registered Nurse

We are seeking a Registered Nurse with a minimum of 2 years of experience for a 13•week contract role as a Home Health Case Manager. The role requires managing home health cases, coordinating patient care, and working with other healthcare professionals.

Work Schedule: Monday to Friday, 8 am • 4:30 pm.

Requirements:
• Valid MA RN License
• Prior experience in Home Health
• Proficiency in OASIS and HCHB preferred

The role involves managing a caseload of approximately 30 patients per week, providing home•based care with a 1:1 patient ratio. Orientation will be provided at the Southborough office, and the candidate must be willing to cover the Needham territory.
