Job Details

• Bridge to Home (BTH) provides treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) ready for discharge but lacking stable housing.
• Assess patients via interviews, chart reviews, and referrals.
• Educate and teach patients based on diagnosis and need.
• Administer medications as needed.
• Initiate nursing interventions including patient/family instruction on treatment, medication, diagnostics, and follow-up.
• Observe and identify underlying factors such as socio-economic, cultural, physical, and language barriers.
• Assist in maintaining accurate medication lists and patient understanding.
• Coordinate response to medical and psychiatric emergencies.
• Schedule nurse follow-up visits and assist with specialty service access.
• Triage patients and perform vital sign checks.
• Document observations, evaluations, and interventions in medical records.
• Participate in preventive health measures including vaccine administration.
• Administer nebulizer and/or metered dose inhalation therapy.
• Use harm reduction and patient-centered care principles.
• Exhibit sensitivity to culturally and linguistically diverse populations.
• Collaborate with team members to deliver compassionate care.
• Participate in care coordination meetings, staff meetings, and trainings.
• Provide outreach to clients in various settings such as homeless shelters and streets.

Job Requirements

• Experience or prior work in healthcare settings caring for people with homelessness, mental illness (including substance use disorders), or justice involvement strongly preferred.
• Strong desire to learn and care for this population if no prior substantial experience.
• Preferred skills include wound care experience.
• Experience with naloxone (Narcan) administration and patient education desired.
• Excellent written and verbal communication skills.
• Sensitivity to culturally and linguistically diverse patient populations.
