Required

RN licensure for the state(s) you work in
Clean driving record, and valid drivers license and auto insurance

What will you do?

The Registered Nurse (RN) Case Manager plans, organizes and directs hospice care with emphasis on community health education/experience. You will build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

You will complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
You will complete, maintain, and submit accurate and relevant clinical notes regarding patients condition and care given.
Travel throughout the day and sometimes after-hours
Case Managers also work an on-call schedule on a rotating basis.
