Dementia Experienced Caregivers- Monday thru Sunday - 9am to 5:30pm - Stamford, CT Collaborative Home Care is currently seeking a compassionate, reliable and experienced caregiver with a CAN-DO attitude to support an elderly couple (husband & wife) in Stamford. The role covers day shifts (9:00am-5:30pm) 7 days a week, split between two caregivers (one covering 4 days, the other 3 days). We are open to discussing which days work best for you, with the goal of building a consistent weekly schedule for the client. Carry out the plan of care including ADLs such as personal care, bathing, dressing, medication reminders. Companion care: conversation, hobbies, activities, shopping, driving to appointments. Work with minimal supervision, maintaining professional relationship with client in accordance with policies. Establish and maintain effective communication with clients, family members, and co‑workers. Perform other duties as assigned on the personal care service plan.
