We are looking for a pharmacist that strives for excellence and enjoys collaborating with other healthcare professionals to provide comprehensive medication management services. The pharmacist will utilize both their clinical and analytical skills to achieve positive clinical outcomes for our residents. This unique opportunity will be based in our long-term care/post-acute care facilities in the Baltimore, MD area. Travel is required throughout the assigned region.As a clinical consultant pharmacist you will:Provide your clinical and professional expertise on all aspects of pharmaceutical servicesPerform monthly medication regimen and admission reviewsEffectively collaborate with the interdisciplinary team to provide pharmaceutical careLead the monthly Pharmacy & Therapeutics clinical team meetingChallenge and grow your expertise beyond traditional regulatory requirementsRequirements:Minimum one (1) year experience as a consultant pharmacist in LTC/PAC facilitiesBS Pharmacy or PharmD degreeCertified Geriatric Pharmacist (CGP) certification, preferredValid pharmacist license in the State of MarylandExceptional organizational skills and the ability to work independently and collaborativelyBenefitsAs a CommuniCare employee you will enjoy competitive wages and PTO plans.
