Job Title: Community Health Worker
Job Description

This position is for a Community Health Worker and is patient-facing. While no hands-on patient care is required, you will engage face-to-face with patients. The Field-Based Community Health Worker (CHW) supports members' health and independence by conducting visits, assisting with care transitions, and addressing social determinants of health (SDOH).

Responsibilities

• Conduct visits to assess member needs and support care plans.
• Accompany members to medical appointments, housing assessments, and benefit offices.
• Provide support to resolve barriers to care such as housing instability, food insecurity, transportation challenges, and social isolation.

• Bilingual communication skills.
• Experience in community health work and care transition.

• High School Diploma or GED.
• Community Health Worker certification from a California-recognized program.

Work involves traveling within a 25-30 mile range daily, Monday to Friday, from 8 am to 5 pm. This role offers a chance to make a significant impact on the community and work closely with interdisciplinary teams including Social Workers, Nurses, EMTs, and Health Advocates.

Job Type & Location

This is a Contract to Hire position based out of Aliso Viejo, California.

Pay and Benefits

The pay range for this position is $24.00 - $26.00/hr.
