This part-time position (28 hours/week) will have you floating between PPMC and PSVMC , where your skills will shine!
As a Clinical Liaison, you will be the vital link between our home care services and contracted facilities. Your strong communication and interpersonal skills will help coordinate:
You'll act as a clinical resource, educating hospital staff about our services and contributing to patient care plans to ensure smooth transitions for patients and their families following hospitalization.
