Your Responsibilities

• Coordinate and arrange home care services between Providence Home Services and contracted facilities.
• Manage home health, home infusion, and home medical equipment services associated with clinical referrals.
• Collaborate effectively with healthcare teams to ensure seamless patient care.

What We’re Looking For

Required Qualifications:

• Oregon Registered Nurse License upon hire.
• National Provider BLS certification from the American Heart Association upon hire.
• Compliance with background check requirements as per Providence Health and Services-OR policy.

Preferred Qualifications:

• Bachelor's Degree in Nursing.
• 1 year of full-time experience in home health or hospice.
• 1 year of full-time experience in home infusion.
• 1 year of full-time experience in discharge planning or care management.
• Strong customer service skills and experience in home care settings.

Join the Providence Home and Community Care (HCC) team, where over 8,000 caregivers and 2,000 volunteers serve nearly 200,000 people each year. We provide a full range of services for individuals facing chronic or life-limiting illnesses, ensuring they receive the care they deserve.

Working Conditions

This is a part-time position with a day shift schedule, located on-site at our Halsey Building in East Portland. The pay range for this role is $50.21 - $70.04 , with additional compensation opportunities available.
